RELIGIOUS/CHURCH INVOLVEMENT
Beginning with your most recent activities, please list all the activities you have participated in with your Church and how you were involved. The six things that will qualify you are the following activities: Altar Server, Reader, working within the Religious Education program, Choir, Minister of Hospitality, and Extraordinary Minister of Holy Communion. Please include dates with each of these.
COMMUNITY INVOLVEMENT
Please list all community organizations or activities you have been involved in during your high school years, and how you participated. Included in this category are Scouts, sports, etc. Please give all dates and list the most recent one first.
SCHOOL INVOLVEMENT AND HONORS
Please list each school organization or activity you have been involved in during your high school years, and any leadership positions held. Also, list any awards or honors received in school.
